Go to the DHA office and obtain an Application form for altering your name in the Register. The applicant must get an application form that meets his or her needs. If you need to alter your surname, you must get Form BI-85 from the DHA office and fill it out as instructed.

All of the aforementioned applications will take 3 to 4 months to process since the modification must be published in the Government Gazette. When the application is complete, the applicant will be sent an abbreviated birth certificate to authenticate the updated details.

It is quite difficult to change your name without a recorded marriage or divorce on the DHA’s system, or the proper papers (e.g., a divorce decree). The DHA BI-196 Application Form for “Authority to Assume Another Surname” is often filed.
