Please note the following Mandatory documents should be affixed to a fully completed Supplier Database Form for further consideration by the College: Fully Completed and initialled Registration. Original and Valid Tax Clearance Certificate Issued by SARS. Certified Copy of CIPC (Previously known as CIPRO) Registration Certificate. Company Profile. Original Certified copies Shareholders / Directors ID. Cancelled Cheque / Confirmation Letter from Bank. BBBEE Certificate. Original certified copies of Shareholders / Directors / Owners / Members Identity Documents. All Service Providers and/ or Suppliers who were previously or are currently providing Services or Supplies to the Maluti TVET must also re Register, as they will not be automatically Included In the Database. Please Note: Blacklisted companies appearing on the National Treasury database and that are prohibited from conducting Business with Public Entities, shall not be considered. Responding to this Invitation does not mean an automatic registration with Maluti TVET. Only Original Registration Forms will be accepted. Completed forms must be submitted in a sealed envelope marked “Vendor Registration” to Maluti TVET College Corporate Office, Corner of High and Broster Street, Eureka, Bethlehem, 9700, for Attention Mrs Ntshonyane NJ or Mrs Van Heerden, on or before 15 May 2015. Faxed or emailed Forms shall not be accepted.
