Bring the following to your nearest DLTC:
- Identity document (ID) and a copy of your ID, previous South African driving license, or valid South African passport
- Four black-and-white identification photos (Before having photographs taken, check with the DLTC to see how many photos they require.)
- Proof of residential address, such as a utility bill. If the utility bill is not in your name, the bill’s owner must make an affidavit confirming that you live at the address and attaching the utility bill to the affidavit.
- If you live in an informal settlement, you must provide a letter from your ward councilor with an official date stamp proving your residence address.
- Application cost as specified.

- Fill out the form for notification of change of address or particulars of person or organization (NCP).
- You will perform an eye test or have one performed at an optometrist of your choice and bring the results to the DLTC.
