- Section 3 – Upload Application Form and Declaration Form
- Section 4 – Agreement
- Section 5 – Declaration
A. Online Application Form instructions:
- Download Application Form and Save it to computer.
- Minimise web browser and open file in Adobe PDF Reader (NOT IN BROWSER)
- Download Adobe Reader here if you do not have it.
- Complete Application directly on pdf – PRINT to PDF or SAVE as PDF. UPLOAD this file.
- Scanned copies will not be accepted.
B. Declaration Form instructions
- Download Declaration Form and Save it to computer
- Minimise web browser and open file in Adobe PDF Reader (NOT IN BROWSER)
- Download Adobe Reader here if you do not have it.
- Complete Declaration Form directly on pdf – PRINT to PDF or SAVE as PDF. UPLOAD this file.
