SSISA Chronic Disease Intervention Programme

Chronic diseases are a group of diseases that share common risk factors associated with lifestyle choices. These factors, which can increase your risk of chronic disease, include an unhealthy diet, smoking, lack of physical exercise, sedentary behaviour and stress.Besides affecting health and quality of life, chronic disease also accounts for approximately 75% of health care spending. There is, therefore, a definite need for a more holistic and comprehensive intervention to manage and prevent the progression of chronic disease.

The Chronic Disease Intervention Programme offers specific, medically-supervised and medically directed rehabilitation for people with chronic diseases of lifestyle.

THIS PROGRAMME CATERS FOR ANYONE WHO HAS:

  • Cardiovascular disease (includes angina, heart failure, hypertensive heart disease, arrhythmia etc.)
  • Peripheral Vascular Disease
  • Diabetes
  • Metabolic Syndrome
  • Respiratory Disease
  • Cancer
  • Renal Disease
  • Rheumatoid and Osteoarthritis
  • Fibromyalgia
  • HIV disease
  • Frailty and ageing

We understand that each person on our programme is different and that there are different degrees of risk which will determine the optimal treatment programme. Based on the evaluation of your risk profile by our Sports and Exercise Medicine Physicians based at SSISA, together with the Biokineticist, a recommendation will be made to find a programme suitable for your needs in keeping with the American College of Sports Medicine risk stratification (ACSM) which is the most widely accepted scientific screening process.

PROGRAMME OPTIONS

PHASE 1 CHRONIC DISEASE REHABILITATION

  • Phase 1 forms the first component of the medically supervised programme. Your exercise programme is individualised to ensure safe and gradual progression and teaches you to trust your body and create an awareness of your limitations. Patient education is emphasised throughout and is a vital component in order to make a permanent lifestyle change.
  • Option 1: 12 weeks (36 sessions) of medically-supervised chronic disease rehabilitation in a group class setting, with a high staff/patient ratio. Recommended for medium risk members.
  • Option 2: 12 weeks (36 sessions) of individual Biokinetics rehabilitation with a Biokineticist. Recommended for high-risk members.

PHASE 2 CHRONIC DISEASE REHABILITATION

  • The second 12-week component focusses on improving your physical fitness and functional strength, as well as further reducing your risk factors. This part of the programme is designed to allow the patient to eventually progress to exercising in setting with less supervision and more independence, such as the Prime Time programme.
  • Option 1: 12 weeks (36 sessions) of medically-supervised chronic disease rehabilitation in a group class setting, with a high staff/patient ratio. Recommended for medium risk members.
  • Option 2: 12 weeks (36 sessions) of individual Biokinetics rehabilitation with a Biokineticist. Recommended for high-risk members.

GROUP CLASS TIMES (FOR PHASE 1 AND 2 SESSIONS)

Monday, Wednesday, Friday mornings from 07:00 – 08:00

Or

Monday, Wednesday, Friday afternoons from 14:00 – 15:00

GROUP CLASS TIMES (PRIME TIME)

Monday – Friday from 07:00 – 08:00

Or

Tuesday and Thursday from 11:15 – 12:15

A “Risk stratification and preliminary consultation” with the Sports and Exercise Medicine Physician may be booked and paid for at the Cape Sports Medicine practice, located on the Ground Floor of the SSISA. Alternatively, you may ask your cardiologist or physician to perform the relevant consultation and complete the necessary forms. At the completion of each phase of the Chronic Disease programme, you will require a re-evaluation. This is also required at 12-month intervals for the Prime Time programme.

ACCOUNT PAYMENT:

This programme is contracted OUT of medical aid, which in essence means that the patient is responsible for his or her account.

If you are covered by medical aid:

A receipt is issued after your payment, which you can submit to your medical aid in order to gain reimbursement. All Chronic Disease Rehabilitation falls under the code of Cardiac rehabilitation, and the tariff code 1431
