TUT Graduation Re-Issuing of Certificates
General information on qualification already obtained
Application for the issuing of a duplicate qualification statement.
An original qualification will not be re-issued to a student in cases where the original document has been lost, damaged or destroyed. Only a duplicate qualification statement will be issued.
In the case of an original certificate being lost or destroyed, the applicant must submit an affidavit (a sworn statement that the certificate has been lost/stolen/destroyed, made at a South African Police Services Station).
Complete the application form
A fee of R211 per qualification must be paid into the following bank account number:
